You Do Not Really Warm Up Before You Lift

The fault

Walking in and going straight to the bar.

Why it happens

Warming up for weightlifting is not general cardio. It is rehearsing the positions you are about to load, so the first working set is not also the first time you visit them.

The fix

One drill that covers the ground a weightlifting warm up needs to cover. Short enough that you will actually do it.
